If you or your child’s primary care provider have completed an autism screening<\/a>, and the results indicate your child is at risk for ASD, you should be referred for a formal autism evaluation, whether it is with Thrivergent Behavioral Services, LLC, or another provider.  At this time, we are only doing evaluations for children ages 10 and under.  If your child is over 10, your primary care provider can refer you to a different provider to start that process.

In order to receive an evaluation through Thrivergent Behavioral Services, LLC, a referral MUST be made from your child’s primary care provider.  To be considered for a referral, we must receive the following from the provider:

1)  Referral form (found here<\/a>) sent from your child’s primary care provider
2)  Results of autism screening
3)  Hearing exam results
4)  Release of information from primary care provider giving us permission to exchange information

Once we have received all the information, we will contact you to complete the necessary paperwork.  

Our autism evaluations are private pay only, and the cost is $1500.  We require $500 down when you make the appointment and the rest to be paid at the time of the evaluation.  We accept a variety of payments, including debit\/credit, HSA, and PayPal Credit.  At the completion of the evaluation, you will be given paperwork that you can submit to your insurance for reimbursement.  Your individual insurance company will then decide if and how much they will reimburse you.
